=== Gut microbiome === In several chronic liver diseases, the gut microbiome, which regulates both the innate and adaptive immune systems, is implicated. This can result in abnormal immunological development and an accumulation of primary bile acids. Using this information a bile-acid–intestinal-microbiota–cholestasis triangle is thought to be involved in the pathogenesis of PBC and PSC. After all, bile acids do modulate the gut microbiota; a disturbance here can result in development and progression of cholestasis. This information has prompted researchers into manipulating the microbiota via antibiotics and probiotics for new treatment options. Some antibiotics examined for PSC include vancomycin, which has extensively studied and reviewed. == Mechanisms of action == Levonorgestrel is a progestogen, i.e. a progesterone receptor agonist. The hormonal IUD's primary mechanism of action is to prevent fertilization. The levonorgestrel intrauterine system has several contraceptive effects, although thickening of the cervical mucus appears to be the primary effect. Other effects include making the inside of the uterus become fatal to sperm and thinning of the endometrial lining, but this is not the usual function. Ovulation is not inhibited in all cases. Numerous studies have demonstrated that IUDs primarily prevent fertilization, not implantation. In one experiment involving tubal flushing, fertilized eggs were found in half of women not using contraception, but no fertilized eggs were found in women using IUDs. IUDs also decrease the risk of ectopic pregnancy, which further implies that IUDs prevent fertilization.
=== Passover === The Jewish festival of Pesach (Passover) may present problems with its obligation to eat matzah, which is unleavened bread made in a strictly controlled manner from wheat, barley, spelt, oats, or rye. In addition, many other grains that are normally used as substitutes for people with gluten sensitivity, including rice, are avoided altogether on Passover by Ashkenazi Jews. === Guidelines === International guidelines were published in 2005 to govern home monitoring of oral anticoagulation by the International Self-Monitoring Association for Oral Anticoagulation. The international guidelines study stated, "The consensus agrees that patient self-testing and patient self-management are effective methods of monitoring oral anticoagulation therapy, providing outcomes at least as good as, and possibly better than, those achieved with an anticoagulation clinic. All patients must be appropriately selected and trained. Currently, available self-testing/self-management devices give INR results which are comparable with those obtained in laboratory testing." Medicare coverage for home testing of INR has been expanded in order to allow more people access to home testing of INR in the US. The release on 19 March 2008 said, "[t]he Centers for Medicare & Medicaid Services (CMS) expanded Medicare coverage for home blood testing of prothrombin time (PT) International Normalized Ratio (INR) to include beneficiaries who are using the drug warfarin, an anticoagulant (blood thinner) medication, for chronic atrial fibrillation or venous thromboembolism." In addition, "those Medicare beneficiaries and their physicians managing conditions related to chronic atrial fibrillation or venous thromboembolism will benefit greatly through the use of the home test."

At 7:59 a.m., American Airlines Flight 11 took off from Logan International Airport in Boston. About fifteen minutes into the flight, five hijackers armed with boxcutters took over the plane, injuring at least three people and possibly killing one, before forcing their way into the cockpit. The terrorists also displayed an apparent explosive and sprayed mace into the cabin, to frighten the hostages into submission and further hinder resistance. Back at Logan, United Airlines Flight 175 took off at 8:14 a.m. Hundreds of miles southwest at Dulles International Airport, American Airlines Flight 77 left the runway at 8:20 a.m. Flight 175's journey proceeded normally for 28 minutes until 8:42 am, when a group of five hijacked the plane, murdering both pilots and stabbing several crew members before assuming control of the aircraft. These hijackers also used bomb threats to instill fear into the passengers and crew, also spraying "tear gas, pepper spray or another irritant" in the cabin to force passengers and flight attendants to the rear of the cabin. Concurrently, United Airlines Flight 93 departed from Newark International Airport in New Jersey; originally scheduled to pull away from the gate at 8:00 a.m., the plane was running 42 minutes late. At 8:46 a.m., Flight 11 was deliberately crashed into the north face of the World Trade Center's North Tower between the 93rd and 99th floors. The initial presumption by many was that it was an accident.
